Preapplication as needed. In order to participate in the
program, the potential eligible employee must complete this form,
check at least one criteria that applies, and sign the form. A
eligibility category must be selected or the form will be returned
for completion. If there is no means of obtaining the
necessary documentation, as a last resort to obtain a credit
voucher, an Applicant Statement can be used for verification of
documentation. The copy has been provided in pdf format below.
