Renewals by Fax:

You may print and fax your membership information by clicking the appropriate form link below.  Reminder:  you do not need to complete the online form unless information has changed.  To pay by credit card and you choose not to use the paypal service, email mhansen@syix.com and request a membership call back to process your credit card payment.   Note:  It is the policy of CAEZ NEVER to open PayPal accounts.
