Membership by Fax:

You may print and fax your membership information by clicking the appropriate form link below.  To pay by credit card and you choose not to use the paypal service, email mhansen@syix.com and request a membership call back to process your credit card payment.   Note:  It is the policy of CAEZ NEVER to open PayPal accounts.
